Immunizations
FALL IMMUNIZATION EXCLUSION DAY
For the 2025-2026 School Year is: October 6, 2025.
Official/valid immunization documentation must be provided to the Kansas City, Kansas School District. Present immunization information at enrollment, building registrar or school nurse.
Any student attending Kansas City, Kansas Public Schools must be in compliance with the State's current immunization laws and regulations. Any student who does not meet the State's requirements may be excluded to attend school on or after October 7th until one of the following documents is provided to the school nurse:
- Up to date immunization records or a scheduled plan to give shots needed signed by the physician

- Signed Medical or Religious Exemption according to the provisions of the law
- USD 500 Religious Exemption forms are required annually by the exclusion due date.
- Medical Exemptions must come from your healthcare provider. Medical Exemption Form

- PCV (Pneumococcal)
- HIB (Haemophilus Influenza Type B)
Elementary:
- DTaP
- Polio
- MMR
- Hepatitis A
- Hepatitis B
- Varicella
Secondary:
- DTaP
- Polio
- MMR
- Hepatitis A
- Hepatitis B
- Varicella
- Tdap (required for seventh grade or higher)
- Meningococcal A,C,W,Y (first dose at 11 years of age and less than 16, second dose at 16+ years of age)
